Neighborhood SHOPP Case Management Program (CMP) funded by The New York City Department for the Aging focuses on planning, arranging and coordinating in-home services to maximize the functional independence, economic and social wellbeing of Bronx homebound older adults residing in community Districts 1-6. This service includes a comprehensive in-home assessment by trained social work professionals for home delivered meals, EISEP homecare and supportive services such as monitoring, advocacy, benefits and entitlements counseling and follow-up on referrals and service provisions.
If you answered yes to any of these questions then you may be a perfect candidate for a Friendly Visiting Service Volunteer. Your visit on a regular basis to a senior will reduce isolation and provide stimulating activities and companionship.
